“Licensed Group Day Care”
Provider Child Care Agreement
(Including Policies & Regulations 2023)

I am Pam Bleigh residing at 2119 Jacqueline Drive, Denton, Texas 76205. Telephone number (940) 453-9867 , herein referred to as the Director.

Fridays 6:00 a.m. -5:30pm.

Fees for Full Time Students
For the above listed hours of care, we agree to pay $130.00 per week. (If your child is between 22 months and two years of age, you will be asked to pay $135.00 a week. After your child has reached the age of two, he or she, will start paying the regular fee of $120.00.) You will also need to pay one week’s pay, up front, to be used as a “holding fee”.  When you have given me your two weeks notice that you will be leaving my daycare, I will use the “holding fee” to pay your last week with us. All payments are due on Friday prior to the week of services. Students, who are new, have to attend the daycare for six months before they are eligible to take half pay for their two weeks vacation, in a calendar year. Parent’s vacation time is two weeks a year at half pay. If you do not use your half pay when Miss Pam is on vacation, then you will have to pay full pay during her vacation.  When you vacation, a prior notice of at least two weeks will be required. Director will be paid for two weeks of vacation during the year.  Please pay any fees due prior to your departure for vacation.  Failure to pay may result in loss of your reserved space for childcare

Fees for “Part Time Students” -that stay year round (school kids)
For the above listed hours of care, we agree to pay $65.00 per week, throughout the school year. If the part time person requires a full week at my daycare, he or she will then be required to pay $120.00. Due to school closings during the year, I’ve offered day fees. If the student requires one or two full days at my daycare, it will be an additional cost of $25.00 a day, and $15.00 for a half day. I also provide, drop off and pick up for your child at school. The other rules and regulations of this policy pertain to part time students as well.

Drop In’s
If the parent wants to drop a child off for a day or two, the fee will be $25.00 a day. Three days constitutes a full weeks pay. 

“Part Time Students” that stay all summer
There will be no discount “half pay” on my vacation week ( usually around the 4th of July). Full pay will be required for that week. Follow all the daycare rules as any other student.

Fees are due in advance and payable no later than the Friday prior to the week that your child attends.  A late fee of $5.00 per day will be assessed for everyday that payment is not made, including weekends.

All Payments will be paid by 5:30pm on Friday.

EXAMPLE:  If you fail to pay on Friday by 5:30pm, but pay on Monday you will owe $5.00 for Friday, $5.00 for Saturday and $5.00 for Sunday.  The $15.00 late fee is due Monday morning along with the week’s fee.

Addendum: If you choose to pay the late fee after 12:00pm Monday, there will be an additional fee of $5.00 for that day, and so on with each additional day thereafter.  

Should a child have an extended illness (more than one week), the full weekly rate will be payable for the first week, then half of the weekly rate will be charged for the duration of the illness to hold the child’s place. It will be up to the parent to notify the Director, if the parent wants to use this method of paying.

Althoughfamily daycare is a form of self-employment, unlike other Businesses, there is no room for growth or expansion due to licensing regulations.  I do not receive benefits that most employees take for granted such as: health insurance, personal days, workman’s compensation, retirement/pension plans, unemployment insurance and annual bonuses, vacation pay.  My contractual policies reflect this. Five paid sick days (per 12-month period) are allotted for Director, this includes but not limited to, personal or family illness, family emergency and or death.

Checks will be accepted.  If, however, a check is returned your weekly fee will be accepted in cash only.  You are responsible for any fees assessed to my account.  Return check fee is $35.00. Any monies owed will be paid to Director no later than the next Friday along with the next weeks pay.

 II. Schedule
The first 30 days of care are probationary for the Director, parent and child.  This agreement may be terminated any time during that period.  Otherwise, two weeks written notice is requiredif the child is to be permanently removed from this day care along with two weeks worth of fees.  Except in the case of gross misconduct on the part of either parent or child.  Director will also tender two weeks notice prior to cessation of care.  Your fee will then be due when notice is given.  (Depending on the degree of misconduct, the Director will decide to let the child stay (his or her) last two weeks, with full payment due on each Friday.

A charge of $1.00 per every minute will be charged for late pick-ups anytime after 6:00 p.m. Monday through Thursday and after 5:30 p.m. Friday (a prior notice would be appreciated.)  This will be paid the day the infraction occurs. 

Please do not ask if you owe an overtime charge when you are late. (It is embarrassing for both of us.)

Please understand: My 12-hour schedule is only “child care hours” and does not reflect all of the additional hour’s necessary to provide quality day care.  I am here for you in case of emergency, but I do not want to work “overtime”.  If I am to do so, it must be financially worth my while.  I too have a busy schedule, which must be arranged, around my providing day care services.

The following is the Provider’s paid holiday schedule:
Memorial Day…………May, 26th through 29th, (Friday-Monday) 2023
Vacation………………….June,30th through July,7th, (Providers Vacation) 2023
Labor Day……………….Sept. 1st through Sept.4th, (Friday-Monday) 2023
Thanksgiving Day……..November, 23rd and 24th, (Thursday & Friday) 2023
Christmas Day…December, 25th through 29th, (Providers Vacation) 2023
New Year’s Eve………on a weekend

In case of emergency or illness, Caregiver will make every attempt to help provide substitute care.  However, parents should be prepared with their own backup.

III. Meals
Breakfast, lunch and snack are served each day at no additional charge.  Our meal schedule is as follows (meal schedule does not pertain to infants):
Breakfast:   7:00-7:15A.M. (School Age Children)
Breakfast:   8:00-8:30A.M.
Lunch:    11:00-11:30A.M.
Snack:        3:15-3:45P.M.
[These times may alter some due to picking up children from their schools]

Should your child arrive later than a mealtime, you will be responsible for feeding him/her, except for special occasions or illness requiring a special diet.  Please do not send any food with your child.  This includes, but is not limited to, gum, candy, chips, cookies, sodas, etc.  Arrangements can be made for special days.

I am a participant in a Child Nutrition Program and make every attempt to provide enjoyable and nutritious meals for your child.  I offer a variety of foods.  Your child is encouraged to try new things, but not forced.

IV. Sick Care
If your child is ill to the extent that it interrupts the care of or endangers the health of the other children, it will be necessary for you to make other arrangements for his/her care.  Should your child become ill during his/her day here, you will be notified and we will determine the best course of action concerning appropriate care, which may include the child being picked up from the child care facility.

I will always respect your need to be at work when it comes to an ill child.  I ask, however, that when deciding if your child should be at home, you give consideration to the other children in care and ask yourself how you would feel if your child was here and well and another child was here and ill.

My “bottom line” specifically, for not providing care is fever in excess of 101, vomiting, excessive diarrhea, or any potentially contagious disorder.  Further, there are times when a child is not that ill, but is terribly uncomfortable and really needs some “one on one” that I cannot constantly provide and be fair to the other children.  At those times I will strongly urge you to consider keeping your child at home.

When a child has been ill, they may return to care if:
1. The child has been seen by a physician who has told you he or she is not contagious.  A physician’s slip is required.  Please inform your doctor, that your child is enrolled in a day care program and will need verification of health to return to care.
2. The child has had no fever, vomiting or diarrhea symptoms for at least 24 hours.
3. The child has only a mild cold with no fever.

Obviously, these rules are for the health and protection for all the children in care and their families as well.  They will be strictly enforced.

The State of Texas prohibits me from keeping ill children.

I will not be administering Prescribed Medicine to any children in my daycare. Please be sure to give dosage before and after daycare (if need be).  Non-prescription medicine, if needed, will also be administered by Director if provided by parent. I would feel better not giving any medicine at all.

Medication Release Form

I will give permission to Pam Bleigh to give my child the appropriate medicine for a mild illness.

I have two cats and they receive shots every year and they do not have their front claws. If your child or children are allergic to cats, please inform me. The children seem to love the attention the cats give them and vice versa.

V. Potty Training
Potty trained: The child is capable of using the toilet, but it is the adult who is trained to get the child to the bathroom on time.  Accidents occur often.  It is the first step in the total process.

Potty Learned: The child is not only capable of using the toilet, but has the developmental ability to express the need to go.  Accidents occur, but infrequently.

Potty training will begin when your child is ready, and that will make the task  easy and quick.  If a parent undertakes the task of potty training when they want it to happen, the task is arduous and painful for all concerned.  Twenty four to thirty months of age is a good rule of thumb to start checking for signs of readiness.  When we agree that the time is right for your child and until he/she is totally successful in his/her toileting, no wearing of overalls or clothing with suspenders, snaps, buckles, belts or zippers.  Elastic waist pants or dresses are preferred.  Any thing else sets your child up for failure.  In addition, your child’s “Personal Bag” must have several, complete change of clothes, in case of an accident. I prefer Pull-Ups over diapers during training period, easier for your child to help with their training. If your child does not have immediate success, he/she will be put back into diapers and after a reasonable amount of time, the process will be attempted again.  This does not harm the child in any way; in fact, keeping a child in “grown-up” pants when they are not successful can be harmful.

VI. Parent Responsibilities
Parents are responsible for providing:
1. Diapers/baby wipes/special creams or ointments necessary for diaper changing.
2. A complete change of clothes (including extra socks) labeled with your Child’s name or initials, appropriate for the weather and maintained in one of Miss Pam’s “Personal Bag”.
3. Proper foot wear, please no shoes with heels.  Sandals and tennis shoes are perfect. Always wear socks.
4. Please provide one toothbrush and one hairbrush with your Childs name on it.
5. Please call no later than 8:30AM to let me know if your child will be attending that day, due to scheduled activities.
6. State Law states, no child should be in any daycare facility for more than 10 hours any given day.
7. Please do not park in front of the stairs leading to my house. There is not another way to get to my front door from the front yard. Courteousness is contagious.

I provide an ample supply of toys, games and books.  Except for a favorite blanket or other special item, I prefer that your child not bring anything from home.  If this cannot be avoided, the parent will accept responsibility for the item. If your child breaks a window or damages property, such as our 52” TV through misconduct, you will be held responsible for the replacement or repair of the property.  This does not include the small toys the children play with daily, but will include the larger, more expensive toys and equipment.

VII. Miscellaneous
Any changes in personal address or phone numbers will be given to the Director as soon as possible.  Names and phone numbers on emergency form will be kept current.

Note that your child will only be released to persons other than the parent or legal guardian if their name appears on your signed emergency form.  In case of emergency, a phone call will suffice as long as that person picking up the child shows current identification.

You and or Spouse are always welcome to come and visit your child or children at the daycare. With or without notice, my house is always open to the parents.

VIII. Contract Changes
Two weeks notice will be given by the Director prior to any significant changes in this contract.  Contract will be renewed annually around the first of the year.  Addendum’s will be added any time they are necessary.

IX. Release Agreements
My child has my permission to play on all equipment and participate in all activities at the home of Pam  Bleigh while in her care or Randy Bleigh.

My child may participate in neighborhood walks.

My child may participate in water activities (sprinkler)during warm weather.

Pam Bleigh has my permission to provide transportation for my child on emergentcy only trips away from the home.

My child may stay in Pam Bleigh’s home under care of another adult in case of emergency.

X. Disclaimer
I will not hold Pam Bleigh or her designated substitute responsible in the case of sickness or injury of my child while in her care, except when it is the direct result of the Directors negligence.

XI. Licensed Group Day Care Home Required Documentation
The State of Texas requires enrollment information, immunization records and emergency medical releases to be maintained and readily available for inspection.  All required forms must be completed and returned before the first day of childcare.  This includes Directors forms as well.

ADDENDUM:
All possible care is taken to provide a safe and healthy environment; however, children do have accidents. I am certified in Pediatric CPR and have emergency first aid training. In case of an emergency I will tend to your child, call 911 and notify the parent.  Your child will be taken to, Denton Regency Hospital, if that becomes necessary.  It is incumbent
